- Sildenafil Sildenafil, a well-known and effective treatment for erectile dysfunction, was launched in 1998 by Pfizer. In recent years the patent ended which allowed cheaper alternatives to become available on the market. There are several different manufacturers of sildenafil but they all contain the same active ingredient. Sildenafil tablets contain the active ingredient sildenafil citrate. This is the same drug as found in Viagra. Sildenafil tablets are very effective at treating erectile dysfunction and are often used first-line for treatment of this condition. Sildenafil helps to relax the blood vessels in the penis which helps to increase blood flow to it during sexual stimulation, resulting in an erection that is hard enough for sexual activity. When sexually aroused, a chemical messenger called cGMP is released in the penis. This causes the smooth muscle in the penis to relax, allowing blood to flow to the penis and cause an erection. Sildenafil, and similar drugs, are known in medical terms as a Phosphodiesterase type-5 (PDE5) inhibitor. PDE5 is enzyme which is present in smooth muscle and is responsible for regulating blood flow. Too much of this enzyme can cause a breakdown of cGMP which in turn may result in difficulty achieving or maintaining an erection. Inhibiting the action of PDE5 means cGMP is unable to break down, therefore leading to an erection. Sildenafil doesn’t cause an erection, it just helps the blood vessels relax enough to let blood flow in to the penis more easily, therefore, if you aren’t sexually stimulated or aroused, it is unlikely you will experience an erection.
